Care managers can help with psychiatric referrals, SNAP (Supplemental Nutrition Assistance Program) benefit enrollment, Medicaid enrollment, provide resources for additional food/utility assistance as well as assistance to enroll in health care. 

What is SNAP?

SNAP stands for Supplemental Nutrition Assistance Program, formerly known as the Food Stamp Program. SNAP is a government program that provides a sliding scale of income designated for food to low-wage working families, low-income seniors, people with disabilities, and other individuals with low incomes.

What is Medicaid?

Medicaid

Medicaid provides medical benefits to low-income individuals and families. Although the federal government establishes the general rules for Medicaid, specific requirements are established by each state. In Louisiana, more than one million residents rely on Medicaid for health care coverage, with a significant portion being children under the age of 19. The Louisiana Medicaid Program operates within the Louisiana Department of Health.
