The CARE Center conducts eligibility screenings and initial assessments to residents of Plaquemines Parish who have been identified (before the age of 22) as having an intellectual or developmental disability.  The completed assessment is provided to the Metropolitan Human Services District to provide services to those residents who have an Intellectual/Developmental Disability.  Those services may include:  disability-related expenses; respite care for caregivers, community home admissions, nursing home admissions, positive behavioral support services, medicaid waiver services, crisis assistance/psychiatric care, flexible family fund, personal care assistance, or intermediate care facilities/companion care.
